CASE MANAGER / BILINGUAL CASE MANAGER

The Case Manager/Bilingual Case Manager is responsible for supporting the dignity and independence of older persons in need of assistance by assessing their situations, identifying unmet needs, arranging for, coordinating and monitoring services to meet those needs, and advocating with other agencies on behalf of the client.

Compensation: High $30s to Low $40’s depending on degree, licensure and experience

Requirements: A Bachelor's Degree in Social Work or related field plus one-year human service experience. Possession of, or eligibility for, LSW licensure preferred. The ability to work as part of team and knowledge of MS Word required. Bilingual in any of the following languages:  Portuguese, Haitian-Creole, Spanish, Chinese.   Vehicle is required.
