Adult Family Care/Community Living Options Outreach Specialist

 

The Community Relations Specialist is part of the Community Relations team and is charged with driving increased Adult Family Care (AFC) referrals.

Cultivating positive relationships with direct referral sources is central to this role—we want area nurses, social workers, and other referral sources to be aware of the program, and impressed with how our organization prioritizes the well-being of their clients.

Working in conjunction with the Community Relations team, the Community Relations Specialist will identify referral source opportunities, and take the lead on reaching out to these organizations to raise awareness. Depending on the circumstances, this could range from sharing printed materials and discussing the program, to setting up an AFC sponsored presentation.

The specialist will also be responsible for tracking outreach efforts, follow-up, and maintaining relationships on an ongoing basis.  Ideally, the specialist would portray themselves as a point of contact, for if a referral source has questions about if the program could help their client.

The Community Relations Specialist would also have a similar, but much smaller, role in helping to promote the Community Living Options program, and supporting other outreach efforts from the Community Relations team.

 

Qualifications/Basic Job Requirements:

  • Associate’s degree in Human Services, or related field required. Bachelor’s degree in Human Services, Social Work, or related field preferred.
  • At least two years’ experience working with social workers and/or case managers in an elder services setting. Outreach and public speaking experience a plus.
  • The Community Relations team is a fast-paced environment that requires strong organizational and interpersonal skills, attention to detail and record keeping, and the ability to work independently.
