Requirements: BA/BS in human-services related field preferred; HS diploma/GED and 3 years related experience required. Must maintain MAP certification. Experience working with people with psychiatric disability, co-occurring disorders preferred. Valid drivers' license required. Must have access to an operational and insured vehicle and be willing to use it to transport clients. Preference given to bi-lingual/bi-cultural applicants and applicants with lived experience of psychiatric conditions.
Job Description: Work as a member of the Collaborative Action for Recovery (CAR) team to provide community-based rehabilitation, support and supervision services to Community-Based Flexible Support (CBFS) service clients. Provide medication education, administration, and support to clients, including supporting clients' medication-related goals, providing medication-related interventions, supporting clients' health improvement activities, attending medication and medical appointments with clients, and adhering to Medication Administration Program (MAP) policies and procedures. Assist the CBFS Nurse Coordinator in maintaining tracking systems to support adherence to MAP requirements for clients served by the team.
Schedule: Monday-Thursday 8:30AM-4:30PM, Thursday 12PM-8PM, rotational weekend shifts every 5th weekend 10AM-6PM, with Mondays and Friday off.
